Skilled Nursing

When your loved one needs an advanced level of ongoing care, South Port Square provides 24-hour licensed skilled nursing care right on campus at our Harbour Health Center. Here, everyone is like family. We get to know our residents on a deeper level, so we can deliver care programs that truly reflect their lifestyles and preferences. Our long-term care suites are furnished, but we always invite you and your loved one to personalize the space, so it really feels like home. Both private and companion accommodations are available.      

Short Term Rehab

Harbour Health Center at South Port Square offers short-term rehabilitative services in a warm and comfortable residential setting to help you recover after an illness or surgery, such as hip, knee, or other joint replacement procedures. Either private or companion accommodations are available at our rehabilitation center. Winners of the U.S. News Best Nursing Home Short-Term Rehabilitation Award for 2019-2020, our skilled therapy team will work with you to develop an individualized plan to get you back to an active life in your own home as quickly as possible. Because South Port Square is a life care community, residents can transition seamlessly into rehab, if needed, but you don’t have to be a resident in order to take advantage of our rehabilitation services.  

Respite Stays

South Port Square provides relief for you as a primary caregiver by offering short-term respite stays for your loved one. You can be sure we’ll provide the highest level of care while you get a much-deserved break and attend to your own needs. Respite stays are also available for those who need a place to recover after surgery or for those who would simply like to try out our retirement community. What is skilled nursing?
Skilled nursing within a senior living community provides 24-hour licensed nursing care. This area within a community primarily provides long-term care or rehabilitative therapies for patients and residents who need medical or nursing care and is ideal for those with chronic medical conditions or disabilities, or those in chronic pain, who require round-the-clock care and supervision by licensed skilled nursing professionals.
Is a skilled nursing area of a senior living community like a hospital?
The skilled nursing area of a senior living community is not like a hospital.  Many patients and residents utilize this area of care after a hospital stay. While our skilled nursing facility does provide advanced levels of on-going long-term care as well as short-term rehabilitative services, the suites and spaces are designed to be warm and comfortable and reflect a home-like environment.
What's the difference between Hospice and Home Health?
Home health is specialized medical attention that seniors can receive in their home to help maintain or regain function. South Port Square can help you coordinate this kind of care, or provide on-site skilled nursing care.
